Register with us as a New Patient

To register as a New Patient, you must complete TWO forms: the NHS Registration Form and the New Patient Health Questionnaire. Both forms are mandatory. You can complete them both online or in person.

Register Online

The quickest and easiest way to register with us is online. There are just 2 easy steps:

Step 2: complete the New Patient Health Questionnaire

Register in person

If you prefer, you can come into the surgery and fill out the forms at Reception. We can help you with the forms. You do not need any ID or proof of address. Call us first on 01273 000544 to make sure it is a good time to come in.

Alternatively, you can print the forms you need, fill them in at home and then pop them in our letter box which is just outside the entrance to Reception. The forms you need are our ‘New Patient Registration Form’ and our ‘New Patient Health Questionnaire’.

Wellsbourne Healthcare is open and welcoming new NHS patients. We have a fantastic team of doctors, nurses and other staff able to help with your health and wellbeing.

Medical treatment is available from the date of registration. Please contact reception for further information.

Step 1: Complete our NHS Registration Form

Our online registration form will collect your details and details of your previous doctor. This information may be added to your record and will allow us to begin to provide you with medical services.

Step 2: Complete our Health Questionnaire

Our health questionnaire allows us to contact you easily and provide the best care we can for you, including repeat prescriptions
